LONGTIME® Reporting Form

This form enables any individual or organisation to report:

• any doubts regarding the compliance of a certified product;

• any communication or potential misuse of the label;

• an issue relating to the operation of the LONGTIME® programme;

• an issue concerning the involvement of a regulatory body.

The report will be recorded, analysed and forwarded to the relevant body. Submitting a report does not automatically lead to the suspension or withdrawal of the label. It is not necessary to have all the evidence in order to make a report. However, please provide information that is as accurate and verifiable as possible.

In the event of an immediate threat to personal safety, this form does not replace a report to the relevant authorities or services.
